Role Overview
A Home Care Nurse is responsible for delivering personalized, patient-centric nursing care at patients homes, ensuring clinical excellence, safety, and comfort. The role involves continuous monitoring, treatment administration, and patient/family education.
Manipals home care model emphasizes 24/7, one-on-one care in a familiar environment to improve recovery outcomes.
Key Responsibilities
Patient Care & Clinical Duties
- Assess patient condition by taking nursing history and vital signs
- Monitor symptoms, progress, and report changes to doctors
- Administer medications, injections, IV fluids as prescribed
- Provide wound care, catheter care, tracheostomy care, etc.
- Assist in post-operative and chronic disease management
- Support elderly, bedridden, or critical patients at home
Home Care-Specific Responsibilities:
- Deliver personalized care plans at home setting
- Educate patients and families on medication, diet, and recovery
- Assist in mobility, hygiene, feeding, and daily living activities
- Ensure a safe and hygienic home care environment
- Coordinate with hospital team for follow-ups or escalation
Documentation & Reporting
- Maintain accurate patient records and treatment charts
- Document medication administration and observations
- Report emergencies or deterioration immediately
Procedures & Support
- Collect samples (blood, urine, etc.) when required
- Prepare patients for medical procedures or tele-consultations
- Assist doctors during home visits (if applicable)
Safety & Compliance
- Follow strict infection control protocols
- Ensure proper handling of biomedical waste
- Maintain patient confidentiality and ethical standards
Eligibility Criteria
Education
- B.Sc Nursing / GNM (Registered Nurse DNC)
Experience
- 15 years (hospital or home healthcare preferred)
Skills
- Clinical competency (IV, ICU basics, wound care)
- Communication & patient counselling
- Emergency handling
- Compassionate and patient-focused approach
Working Conditions
- Field-based (patient homes across Delhi NCR)
- Shift-based / 12-hour / 24-hour care (depending on case)
- May involve critical care or elderly care cases
