An HCC medical coder specializes in Hierarchical Condition Category (HCC) coding, which groups related diagnoses into categories used in risk adjustment payment models. These coders review patient medical records to identify clinically significant diagnoses that impact a patients risk adjustment factor (RAF), which is used by the Centers for Medicare & Medicaid Services (CMS) or the Department of Health & Human Services (HHS) to calculate payments to health plans based on patient complexity AAPC+2.
