Job Description
Role & responsibilities
The Patient Care Coordinator serves as the primary point of contact for patients and their attendants, ensuring a seamless patient journey from registration to discharge. The role focuses on delivering an exceptional patient experience by coordinating with clinical and non-clinical departments, resolving patient concerns, and ensuring timely service delivery.
Key Responsibilities
- Welcome and assist patients and attendants throughout their hospital visit.
- Coordinate patient appointments, admissions, transfers, investigations, and discharge processes.
- Liaise with doctors, nursing staff, diagnostics, pharmacy, billing, and other departments to ensure smooth patient flow.
- Provide accurate information regarding hospital services, procedures, and policies.
- Address patient queries, concerns, and complaints promptly and escalate complex issues when required.
- Ensure timely completion of documentation and maintain accurate patient records.
- Coordinate with insurance/TPA teams for cashless and reimbursement-related processes, wherever applicable.
- Monitor patient satisfaction and obtain feedback to enhance the patient experience.
- Ensure compliance with hospital quality standards, NABH guidelines, and patient confidentiality policies.
- Support departmental quality initiatives and process improvement activities.
